56 Stars Nomination Form

This form allows individuals to submit their nominations for the monthly '56 Stars' recognition program. You will need to fill in required fields to suggest a 'Star', and 56 Mini Storage will select the winner each month.

  • Please list the full name of the group, organization, or team being nominated for this recognition.
  • Please list the school/organization associated with the above nominee.
  • Please describe why you are proud of your nomination, and any ways they have recently shown integrity, drive, and triumph in their particular arena… Arts, Sports, Academic, Community Service.
  • i.e. parent, teacher, leader

Please note: You must be 21 years or older to nominate a student group for our “56 Stars” program. Selections will be made by our team of judges. The decisions of the judges are final. Specific individuals are not eligible for the 56 Stars award.
